-Dear Patient, I want to introduce you to Dr. Nkem Iroegbu, MD. Dr. Kem (as he goes by) is a hospitalist at St. Joseph Hospital (SJH). I have partnered with him to care for you if you require admission into the hospital. A hospitalist is a physician whose practice is devoted to treating patients in a hospital setting. Dr. Kem will direct and manage your care at the hospital. I will contact him when you require an admission and will forward him your medial history and medications at the time of admission. At the time of discharge, he will contact me and report your hospital course of care with me. It should be a seamless process on your part plus provide you with the highest standard of care while you are a patient at SJH. I will stay in touch with you by phone on most days of your hospitalization to check in on your progress. By partnering with Dr. Kem, I will be able to focus wholly on your outpatient healthcare needs. Dr. Kem has had his hospitalist practice at SJH since 2001. He is also Associate Program Director for the Department of Medicine at SJH and Assistant Professor of Clinical Medicine at University of Illinois College of Medicine. He completed his internal medicine internship at Flushing Hospital Medical Center in New York and his residency at Indiana University School of Medicine and Medical Center Hospitals in Indianapolis. He also received his Master of Public Health at University of Illinois. He is a wonderful person and physician and you will be under great care while at SJH.
